In fact, there is great heterogeneity among individuals of similar weight and BMI in terms of insulin sensitivity; and the underlying molecular factors driving disease progression are currently poorly understood. This lecture illustrates how a personalized omics approach to large-scale longitudinal data can offer a novel insight and new strategies for predicting and preventing disorders of aging, and specifically obesity-associated metabolic disease.

Telomere Technology: End of DNA As We Know It

The ends of DNA—telomeres—have been the subject of intense investigation over the past three decades. We now know that telomeric DNA is profoundly different from the rest of our genome. While there are no polymorphisms of the non-coding telomeric sequence of repeating TTAGGG nucleotides, the number of these repeats (telomere length) is highly heritable, highly variable, and highly malleable over the lifespan. Thus, telomere length is the only part of our DNA that is susceptible to modification by lifestyle, diet, exercise, supplements and drugs. It therefore offers an opportunity at the intersection of genetics and epigenetics to inform precision medicine. In this talk, I will discuss how to apply this emerging telomere science (and some new data) to everyday practice through case studies demonstrating how telomere length can be measured, interpreted, and improved.

Although progesterone is often referred to as the “hormone of pregnancy” as the origin of the name comes from the importance in “pro-gestation,” this hormone goes far beyond its role in placental integrity and maintenance of a pregnancy to full term. While taking a thorough Medical History produces about 80% of clinical decision making, laboratory testing is unarguably a crucial factor in the diagnosis, treatment, and follow-up care of the Interventional Endocrinology patient. During the “Quantum Interventional Endocrinology” lecture, we explored the implicit inability of classical Epidemiology in describing the hormonal landscape. Now with a new perspective of the benefits, limitations, and even the pitfalls of the various forms of laboratory testing, how is the practicing physician able to obtain an accurate picture of their patients’ true hormonal status? The available laboratory matrixes include blood, urine and saliva. Here we—in an unbiased fashion--delve into the claims made by those who provide these forms of testing. We also must appreciate the evolution of our laboratory methodologies, i.e. abandonment of inferior immunoassay techniques for more accurate and reliable mass spectroscopy. And because physicians must “operate” in the real world, how do we strategize the logistics of objective information management: patient lifestyle, compliance, ability to pay, and medical insurance coverage? In response to attendee requests for information on aesthetic protocols and treatments which are office based and can be done by non-board certified Age Management Clinicians either full time or as an adjunct to their existing practice, AMMG presents this track in conjunction with the American Academy of Microneedling. The decision not to accredit this track was made so that attendees could be provided with business tools as well as the science and medical information necessary to implement these procedures. The goal is not to make this track commercial but to give speakers in this niche area the leverage to openly discuss procedures and answer questions that could be severely restricted in an accredited environment.

Thursday, November 2nd Pre-Conference Track 4: Traumatic Brain Injury: A Clinical Approach to Diagnosis and Treatment
This full day program, TBI – A Clinical Approach to Diagnosis and Treatment, will provide to the attendees overwhelming information culled from peer-reviewed articles on Traumatic Brain Injury as the causative factor for personality changes, cognitive impairment and mood disorders leading up to suicide. The attendees will be walked through the literature discussing the effects of primary trauma and the secondary effects of inflammation and oxidative stress on functionality.

Hormone Replacement therapy (HRT) in women has had its ups and downs over the past 35 years. Observational studies in the 1980s and 1990s have shown a reduction in cardiovascular disease (CAD) in postmenopausal women using HRT. The Women’s Health Initiative (WHI) study in 1991 showed an increased risk of CAD while the estrogen only arm of that trial in 2002 showed no effect on CAD. The North American Menopause Society and the international menopause societies all do not recommend use of HRT to decrease CAD. The Kronos Early Estrogen Prevention Study (KEEPS) trial in 2012 from Harvard Medical School showed favorable effects of HRT in newly menopausal women. Other studies performed at the Mayo Clinic showed that HRT might have a role in preventing CAD. The current lecture will review some of the more recent published studies from peer reviewed medical journals. Topics will focus on how HRT is associated with a marked improvement in survival in postmenopausal women with advanced HF; how HRT significantly increases apolipoprotein A-I levels and decreases Lp(a) and the effects on serum markers of inflammation in women with CAD; and how HRT protects against smoking-induced changes in vascular structure and function. Results of a recent study presented at this year’s American College of Cardiology's 66th Annual Scientific Sessions that showed lower atherosclerosis and very clear survival benefits in women using hormone replacement therapy will also be discussed. New Biomarkers to evaluate vulnerable plaques in women to see if they are candidates for HRT will also be addressed.

Starting in 2014, the Warrior Angels Foundation joined with the Millennium-TBI Centers to offer retired Special Operational Forces an alternative approach to their current treatment for Post-Traumatic Stress Disorder. Many were on a polypharmacy that created another form of disability associated with further cognitive and emotional turpitude. We started enrolling Veterans, on a first come basis, into the TBI project where each was drawn for neurosteroid and neuroactive steroid assessment along with a few markers of inflammatory risk (DHEA-s, Vitamin D3, Pregnenolone, Androgens and Estrogens), SMAC24, and CBC. Based upon their results we used the median of the range for each marker to direct treatment. An average of 6 nutraceuticals were used along with Clomiphene citrate and/or natural thyroid replacement when indicated. Enrollees were given the Millennium Program Questionnaire (MPQ) every 30 days as a subjective on treatment and monitoring laboratory testing every 3 months. This presentation will review the literature supporting our approach to TBI/PTS and then present the results of this 3-year study, 2014–2016, on 58 medically disabled veterans who now have their lives back and are off 90% of their medications.

CBD (Cannabidiol) is emerging as a powerful supplement with a significant effect on our body’s endocannabinoid system. The endocannabinoid system has been shown to have a strong effect on the inflammatory, immune, cognitive and motor systems of the body. Cannabidiol (CBD) is one of over 80 cannabinoids, natural compounds found in industrial hemp. CBD and delta-9-tetrahydrocannabinol (THC) are the two main active compounds found in the cannabis plant and are therefore the most recognized and studied. Unlike THC, CBD does not produce psychoactive effects, and it seems to have natural protection against the marijuana high. CBD without THC has shown promising results in independent research and studies as a treatment for a variety of conditions ranging from: Antiemetic,Anticonvulsant Antipsyotropic Anti-inflammator Anti-oxidant Anti-tumoral Anti-anxiolitic/anti-depressant and more. Enteric microbial dysbiosis leading to immune system activation and dysregulation, promote excess adrenal secretion of cortisol, shunting of steroidogenic precursors away from the generation of homeostatically essential anabolic hormones, as measured by decreases in salivary testosterone, progesterone, DHEA, and serum 25OH Vitamin D. Depletion of these hormones and relative excess and dysregulation of estrogens, in the setting of chronic inflammation facilitated by IL-1 and other inflammatory cytokines, and activation of innate lymphoid cells induced by gut dysbiosis, promote metabolic syndrome, increased tendency towards autoimmune pathology, weight gain, prothrombotic states, vascular inflammation, insomnia, chronic fatigue, impaired cognitive function (commonly memory impairment or cognitive fog), inflammatory dermatopathies & arthropathies, gonadal dysfunction, and cyclic menstrual abnormalities. The amount of body fat we carry, in most individuals, is directly proportional to the amount of medications we take. In my practice, 100% of my patients require less medication for hypertension, diabetes and dislipidemia. I will show how testosterone optimization alters our HDL map, lowers our LDL while increasing our HDL. This, in turn, leads to a reduction in statin use. This is true in both men and women.

Mood disorders are an exceedingly common problem in our society today. Given our high stress and constantly changing world, mood disorders can present in many ways and be called by many different labels. Intermittent fasting (IF) is a term for an eating pattern that cycles between periods of fasting and eating. It is not related to which foods you should eat, but rather the timing of when you should eat. In the strict sense it is NOT a diet but rather a “pattern of eating.” Common intermittent fasting methods involve daily 16 hour fasts, or fasting for 24 hours, twice per week. Fasting is one of the oldest forms of “eating” in the world, and modern science has proven that it has many health benefits. Humans have actually been fasting throughout our evolution. At times it was done because food was just not available. It has also been a part of major religions, including Christianity, Buddhism and many others. These religions believe that fasting was very beneficial not only for the spirit but also for healing the body. Our ancestors didn’t have the availability of food all year-round. There were times when they couldn’t find anything to eat, and their bodies evolved and functioned without food for extended periods of time. Fasting from time to time is a more “natural” phenomenon than consuming many meals per day. There are some changes that occur in your body when you fast. The effects on Human Growth Hormone (HGH), Insulin and Leptin sensitivity, Ghrelin normalization, cellular repair, gene expression, biomarkers for inflammation and healthy metabolism, longevity and more will be looked at and studies to support these effects on the human body will be presented and discussed. Clinical cases studies will also be presented, showing how Intermittent Fasting helps in Weight Management, Hormonal balance and Metabolic diseases. In the 21st century, it’s all about metabolic diseases. Many diseases are “dietary diseases.” The problem is that we are trying to use drugs to treat dietary diseases. It is well understood that hormones interact with the DNA in our body to influence metabolic and enzymatic control. Estrogen, for example, increases activity of glutathione peroxidase and decreases inflammatory activity of NFKappaB. As we age, estrogen declines and therefore its favorable influence on these end targets is altered. By managing the decrease in glutathione created when this occurs and the increase in inflammation, we have many other natural interventions that can be used that are less invasive than hormone therapy. By understanding not only hormone testing, but being able to evaluate the environment that hormones are in, we can monitor the physiologic significance of change in estrogen. Not only do these measurements increase clinical understanding, but they widen treatment options for addressing menopausal concerns. Functional medicine is a necessary cornerstone in safe and effective hormonal management and treatment.

The Root Cause of Thyroid Disease

An estimated 12 percent of the U.S. population develops a thyroid condition during their lifetime. 60 percent of those patients are symptomatic but unaware of their condition. Of the newly diagnosed and undiagnosed, fully 90% of them are due to the thyroid attacking itself, i.e. autoimmune maladies. I have lived in Reno, Nevada for approximately three years. I wrote an overview article on hypothyroidism in Healthy Beginnings Magazine in July of 2015 and the response (typically, “Dr. Clearfield, it was if we were in the same room and you were talking to and about me personally) was overwhelming. (http://hbmag.com/patient­mysteries­are­you­thyroid­deficient/). Autoimmune thyroid disease, specifically Hashimoto’s Thyroiditis is practically an epidemic in Northern Nevada. In my adopted hometown, it is poorly understood, poorly diagnosed and poorly treated. Patients, especially women, frequently present with anxiety disorders, depression, and dysthymic disease. The local “standard of care” is to run a TSH and if it is “normal,” treat the patient with antianxiety agents and antidepressants. Much to the chagrin of the local endocrinologists—one even wrote in a medical chart that I was a “shaman and a disgrace to my degree”—I am considered the thyroid expert in Reno, Nevada.
